I, _____________________________, (Adopter), hereby swear and attest that the following information is true and will be followed to the best of my abilities concerning the adoption of the pet listed above (hereafter referred to as “the pet”):

1. I am adopting the pet solely as a domestic pet for myself and/or my family. I will not use the animal for any illegal activity including fighting.
2. I will not physically abuse, neglect, starve or harm the pet in any way.
3. If the pet is not fixed, I will either get it fixed or else ensure that the pet’s offspring receive proper care and a good home.
4. I will keep my pet up-to-date on all vaccinations.
5. I will make sure that the pet has enough food and water to maintain a healthy weight.
6. I will make sure that the pet has adequate shelter.
7. I will ensure that the pet gets the necessary requirement of daily exercise.
8. I will never let my pet outside unless it is wearing proper identification.
9. I will obey all laws concerning leashes and waste removal.
10. I will train the pet so that it does not harm others.
11. If the adoption does not work out, I agree to offer the pet back to the seller before seeking other placement. The seller will have __________ days to reacquire the pet.
12. I will pay the fee of $______________ in order to cover the cost of initial immunization, deworming, neutering/spaying and pest prevention.
