_______________________ (hereafter “Lessor”) and ___________________________ (hereafter “Lessee”) entered into a lease agreement starting on {MM/DD/YYYY} that was set to terminate on {MM/DD/YYYY}.

The Lessor conveyed the Lessee with the premises at:
