- {Company Name} agrees to eradicate the pests from {Client Name}’s {home, office, etc.} located at {address of location}.
- {Company Name} will begin initial inspections on {date} and finish procedures by {date}.
- {Company Name} will only remove the following pests: {Pests}
- {Company Name} is NOT equipped to remove the following pests: {Pests}
- {Company Name} will inspect the premises for signs of damage. They will use {Materials or Procedure} to test for other types or breeds of pests.
- {Company Name} will use the following materials in the process of controlling or eradicating these pests: {list of all materials to be used in the process}.
- {Company Name} will monitor the premises for {amount of time} to ensure that no pests survive or return.
- {Client Name} agrees to pay {Company Name} a total of {amount in dollars} for the pest removal, payable {details of any payment plan}.
- {Company Name} agrees to notify {Client Name} if there are any changes to the materials used or date of completion, and to not continue with any work until {Client Name} approves the changes.
- {Company Name} agrees to remove all materials and debris from {location} upon the completion of work.
